Tentative Schedule: (classes are one hour in length and session is six weeks long)

Afternoon Classes

Agility Novice/Pre Competition 12:00 fee $115 (class limit is 6 dog and handler teams)

Agility Novice/Pre Competition1:15 fee $115 (class limit is 6 dog and handler teams)

Agility classes are $115.00 for a six-week session. Please fill out form and send payment to me, Louise Wetzel, to reserve your spot in the class. Payment and completed application form must be sent to me in order to hold your spot in the class.

Class cancellation policy: Spring 2009 session, if class will be canceled due to weather conditions, there will be a message on the machine at Dog Works by noon on that Monday morning advising you of the cancellation. Please call and check if you are at all concerned about the possibility of class cancellation. This is the number to call and there will be a message on the machine: 631-218-3466

LIE EXIT 61 SOUTH. Proceed to 4th light after South Service Road. This is Broadway Avenue. Make right onto Broadway Avenue. Proceed around bend staying on Broadway. Continue past elementary school. You will come to a set of two traffic lights by a 7-11. Dogworks is the second building after the 7-11 on the right hand side of the road. It is right behind Total Pet Care.

SUNRISE HWY EASTBOUND EXIT 50--LINCOLN AVE/BROADWAY AVE. Proceed on service road crossing Lincoln Ave. Next light is Broadway Avenue. (Attias Flea Market on right) Make a left onto Broadway Avenue. Proceed north crossing Rte 454/Vets Hwy. Dogworks is second building on the left.

SUNRISE HWY WESTBOUND EXIT 51--NICHOLS ROAD/RTE 454 VETS HWY. Proceed on service road going under Nichols Road overpass. Bear right onto Rte 454. At first light-Broadway Ave-make right. Dogworks is second building on the left.

INDEMNIFICATION I understand that attendance at dog agility training class is not without risk to myself, members of my family and/or guests who may attend, or to my dogs. Some of the dogs to which I will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest of care. I understand that agility obstacles by their very nature present a potential danger for dogs and/or handlers, notwithstanding care taken to insure safety. I have examined the training facility and the surrounding area and accept all risks associated with working with my dog in this facility. I hereby release and hold harmless [Dog Works Form and Function], its officers, members, instructors and students from any and all liability of any nature, including, but not limited to, any injury or damage which I, my family members and/or guests, or my dog(s) may suffer while attending any training sessions or other function of the class, or while on the training grounds or surrounding area, including, specifically, but not limited to, any injury or damage resulting from the action of any dog, including my own, and I expressly assume the risk of any such damage or injury.

Accordingly, my signature as set forth below represents my agreement to indemnify and hold harmless [Dog Works Form and Function], its officers, members and instructors, from any and all claims by any person including claims by any member of my family or any other person accompanying me to any training session or function or arising while on the grounds or the surrounding area thereto as a result of any action, negligence, or omission by any person or dog, including my own.
